I hereby certify that the information on this application is accurate, true, and correct. I understand that any false answer or statement or misrepresentation by omission made by me on this application, or any other related document(s), will be sufficient grounds for the rejection of my application or the immediate disqualification of my membership should such falsifications or misrepresentations be discovered after my acceptance.

I hereby knowingly and voluntary authorize Braddock Heights Volunteer Fire Company to investigate my personal history, including but not limited to my employment, military service, education, Motor Vehicle Association (MVA) records, and criminal records, and authorize all persons and entities possessing information related to my past history to provide that information to the Braddock Heights Volunteer Fire Company. I also agree to execute all forms and to take all steps needed for the Braddock Heights Volunteer Fire Company to secure such information. I furthermore agree to hold harmless Braddock Heights Volunteer Fire Company, their service provider, agents of, and any governmental agencies providing information and/or reports about me from any and all liability arising out of the release of any such information and/or reports.

I understand that all information that I provide in this application for membership is considered confidential and will only be released with my written consent, or, as I have authorized within this application.

I agree that if I am accepted into the Braddock Heights Volunteer Fire Company that I will abide by the Constitution and By-laws of this corporation and will follow all standard operating procedures either in place or revised. I agree that I will be a probationary member for a period of one (1) year from my acceptance. I furthermore understand that while a probationary member, the Braddock Heights Volunteer Fire Company reserves the right to disqualify my membership at any time, for any reason, with or without notice.
